CEOGC News: CEOGC Offers Prescription Assistance Program

Beginning November 16th, The Council for Economic Opportunities in Greater Cleveland (CEOGC) will offer the Health and Social Empowerment program for families who are in need of financial assistance to purchase prescribed medications.  The agency is now accepting applications at its five Neighborhood Opportunity Centers. 

 

Those seeking financial assistance for prescription purchases, must be willing to submit to the application process which includes determination of their eligibility for other state programs by accessing the Ohio Benefit Bank and determination of their eligibility for aid from pharmaceutical companies by accessing the application process for the Partnership for Prescription Assistance.  CEOGC will grant up to $150 for prescription assistance (exceptions may apply). 

 

Only those families or individuals with household incomes at or below 200 percent of the federal poverty income guidelines will be determined eligible for this assistance.  The Health and Social Empowerment program is funded by a 2009 American Recovery and Reinvestment Act allocation to the federal Community Services Block Grant, administered by the Ohio Department of Development.

 

To make an appointment to apply for benefits of the Health and Social Empowerment program services, Cuyahoga County residents should call (216) 518-4010.  The appointment scheduling line is available 24 hours daily.    Medical providers and hospital social workers should call CEOGC’s office at (216) 696-9077, extension 231 for other instructions for emergent needs.
